Observation

Shelf in play area was observed to be unstable and not secured casuing a falling hazard. Provider's stove door in food preparation area is broken. Provider's cabin door for kitchen counter and under bathroom sink are broken. Provider has extension cords plugged in for long period of time, as it is plugged in for the tv. Provider has one extension cord plugged in to an outlet next to the bathroom sink.
